List of Accepted Documents

A membership card for the organization that includes either an expiration date or notes “Active” status. (Please note that a professional ID or employment ID that does not include information about membership cannot be accepted.)
A pay stub showing dues paid. Please blackout all information other than name, pay date and the line item denoting dues paid.
A letter on letterhead with signature from the organization stating that you are a member in good standing.
A receipt showing annual dues paid for the current year.
DD214 or proof of current military service.
